Hydrotherapy and biokinetics — water-based rehabilitation explained

Hydrotherapy — also called aquatic therapy or water-based rehabilitation — uses the unique properties of water to support movement, reduce pain and restore physical function. At Gabbio Biokinetics, hydrotherapy is a fully supervised, exercise-based programme led by Gabriella Moreira, a HPCSA-registered Biokineticist. It is rehabilitation, not swimming lessons.

What makes water different?

Three properties of water make it ideal for rehabilitation when land-based exercise is painful or difficult:

  • Buoyancy — water reduces the effective weight bearing on joints. At chest depth, you bear roughly 25% of your body weight. This allows people with severe joint pain, arthritis or recent surgery to exercise with a fraction of the load they'd face on land.
  • Resistance — water provides multi-directional resistance proportional to how fast you move. This makes it an excellent medium for building strength without heavy loading.
  • Warmth — warm water relaxes muscles, reduces spasm and increases circulation — making movement easier and reducing pain during the session.

The result is an environment where patients who struggle to walk comfortably on land can move, strengthen and rehabilitate in a way that would be impossible elsewhere.

Who benefits from hydrotherapy?

Hydrotherapy at Gabbio Biokinetics is appropriate for a wide range of patients, including:

  • Arthritis and joint pain — osteoarthritis of the knee, hip or spine responds exceptionally well to water-based exercise, which builds supporting muscle without compressing already compromised cartilage.
  • Post-surgery recovery — after joint replacements, spinal procedures or lower limb surgery, hydrotherapy allows early movement and strengthening while the body continues healing.
  • Chronic pain conditions — fibromyalgia, lower back pain and conditions where land exercise provokes flare-ups respond well to the reduced-load environment.
  • Neurological conditions — stroke rehabilitation, Parkinson's disease and multiple sclerosis all benefit from the supported, stable medium water provides for movement retraining.
  • Older adults and fall prevention — balance retraining and lower-limb strengthening in water gives older adults confidence to work on stability without the fear of a land-based fall.
  • Low-impact conditioning — patients who need cardiovascular exercise but cannot tolerate impact (due to obesity, joint disease or injury) can train effectively in water.

What does a session look like at Gabbio Biokinetics?

Sessions are held in the pool at St. John's College in Houghton, Johannesburg. Each programme is specifically designed by Gabriella based on your medical history, current condition and rehabilitation goals — this is not a generic fitness class.

A typical session includes:

  • A warm-up phase with gentle range-of-motion work
  • Targeted strengthening exercises using water resistance and equipment (pool noodles, floats, resistance bands)
  • Balance and proprioception exercises where relevant
  • Cardiovascular conditioning at appropriate intensity
  • A cool-down and stretch to close the session

Sessions are progressive — exercises become more challenging as your condition improves. Gabriella monitors your form, intensity and progress throughout.

Group classes or individual sessions — which is right for you?

Hydrotherapy at Gabbio Biokinetics is offered in two formats, and Gabriella will help you decide which suits your situation:

Group classes

Small group classes follow a shared progressive programme and are ideal for patients with similar conditions (such as an arthritis group or a post-surgery recovery group). The group format provides structure, motivation and social connection — and is typically more affordable than individual sessions. Groups are kept small so that Gabriella can monitor each participant and adjust exercises individually where needed.

Individual sessions

One-on-one hydrotherapy is tailored entirely to your specific condition, goals and pace. It is the better choice when your condition is complex, your needs differ significantly from a group setting, or when you need more intensive hands-on guidance. Individual sessions also suit patients who are in an early or sensitive phase of post-surgical recovery.
